FOREIGN MERCHANT SHIPS UNDER DETENTION IN UK PORTS SHIPS DETAINED IN SEPTEMBER 2005
Date & Place of detention: 05/09/2005 - Blyth Vessel Name: LITTLE STAR (General Cargo) 1697 GT IMO No: 8897813 Flag: Georgia Company: J-S Co “FIRM”, St Petersburg, Russian Federation Classification Society: Russian Maritime Register of shipping Summary: Detained for 26 days. 26 recorded deficiencies (5 of these detainable and 2 Major non-conformities raised under ISM Code). Master of vesselin a drunken state and not capable for duty and had to be removed by police. As a result manning not as required on Safe Manning Document. Prohibition Notice also issued for non provision of any gangway. Unable to seal engine room which was open to atmosphere and cargo spaces - emergency escape hatch cover missing, top of port side funnel damaged and skylight incapable of closure because plates wasted. Rescue Boat deflated and not ready for immediate use, and davit inoperable. Emergency Preparedness inadequate, and Maintenance of Ship and Equipment not in accordance with onboard Safety Management System.

Starboard Lifeboat not launching properly - unable to launch for 15 minutes. Engine room fire dampers seized. GMDSS Radio reserve source of energy deficient. Safety and Environmental policy has not been maintained at all levels of the organization, as required by ISM Code. Deep freeze not working and insufficient food onboard for intended voyage. Laundry facilities not working. Weathertight doors including those to paint locker, CO2 Room and port side accommodation incapable of closure. Means of escape from aft accommodation exits blocked by stores and containers. Several minor ISM non conformities also raised e.g. Crew unfamiliar with fire and abandon ship signals even thought records indicate they had completed familiarization as per SMS. Also Senior Officers not familiar with CO2 Instructions. Released on 15/09/05 for a single voyage to a repair port in Istanbul.

Unable to start port side lifeboat engine. Engine room dirty – deck plates covered in oil and numerous drip trays collecting leaking oil from pipes and valves. Port Funnel fire dampers inoperative The Safety Management System on board the ship had failed to identify and rectify the deficiencies.

Emergency fire pump inoperative. Satellite EPIRB inoperative and SATURN Equipment inoperative.

Port and starboard lifeboat launching appliances substandard - winch holding down bolts heads severely wasted, and lifeboat falls blocks cheek plates badly wasted. Maintenance of ship and equipment substandard. Emergency preparedness poor and not in accordance with SMS indicating a lack of training for the crew. Fire drill substandard –no fire plan used on the bridge, crew entered area of drill without appropriate breathing equipment, or safety equipment, and the fireman did not have fire hose or second fireman with him.

Vessel unsafe due to condition of ship’s hull structure, deck transverse beams holed with corrosion, numerous side frames distorted due to operational damage, vessel fails to meet the stability requirements for a vessel loading bulk grain, evidence of insect infestation in cargo hold.

Unable to close fuel fire dampers due to defective control wire and numerous other fire dampers defective. Sickbay in an unhygienic condition with no running water to toilet, bath or sinks and all taps in an extremely dirty condition. Port lifeboat lifting pin and starboard lifeboat lifting lug found to have significant wastage and corrosion. The company had failed to implement procedures to ensure the ship is maintained in accordance with the relevant legislation.

Rudder trunk air pipe detached from deck, Nos. 1 and 6 air pipes corroded through, No 3 and 5 ventilators corroded through. Known structural deficiencies had not been reported to the Company

Released from Detention on 09/09/05. 60 recorded deficiencies (13 detainable). MF/HF Radio Installation had no antenna. INMARSAT had no 24V supply, MF/HF Radio had GPS I/P missing. EPIRB incorrectly mounted. Various fire doors either not closing properly or holed. Decks show signs of corrosion. Anchoring devices, winches and capstans corroded. Lifeboat bilge pump to fit and test. Fire and Boat drills unsatisfactory. Courses in navigation records are in contravention of COC Regulations. Fuel oil purifier and pipe work to clean in engine room. Floor plates around boiler to secure. Galley hot plate to clean. Meat /fish cold room door not closing properly so room above required temperature. Sanitary facilities unhygienic. Various communal toilets and bathrooms facilities not working. No hot water available in Galley or in any sinks in accommodation. Signs of cockroach infestation in accommodation area. Gangway corroded and structurally damaged. No evidence of planned maintenance system working i.e. Audits and corrective actions being carried out.
